Alternate Solutions Health Network is hiring a Registered Nurse for home health care. The RN will provide skilled nursing care in patients' homes, educate families, and ensure safe, compliant care under physician orders.

Formación

• RN license in Indiana is required.
• Minimum of one year experience as an RN in an acute care setting; home care experience preferred.
• Ability to communicate effectively with patients, families, physicians and coworkers.
• Detail-oriented, compassionate, and observant with strong nursing judgment.

Responsabilidades

• Provide skilled nursing care in patients' homes as ordered by the physician.
• Observe and monitor patient conditions; perform OASIS assessments when applicable.
• Administer medications as prescribed and coordinate care with the interdisciplinary team.
• Educate patients and families about disease processes and care plans.
• Ensure accurate documentation per agency and Medicare guidelines.
• Maintain active RN license and comply with state regulations.
