Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse to deliver one-on-one, in-home nursing care. You will work with a dedicated clinical team to manage personalized care plans and support patient recovery in the home setting.

You will perform skilled nursing tasks, monitor progress, educate families, and coordinate with other health professionals. A valid RN license, CPR, and driving to patient homes are required.

Qualifications

• Must have med-surg, ICU, ER, or acute care experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Provide skilled nursing care in patients' homes, administer medications, wound care, and patient assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patients' conditions and report changes to physicians/Clinical Manager.
• Educate patients and families on disease management and treatment options.
