Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse to provide one-on-one, in-home care and manage individualized care plans. You will work with physicians and clinicians to support recovery and help patients regain independence in their own homes.

Responsibilities include administering medications, wound care, patient assessments, education for patients and families, and coordinating with the care team. A valid RN license and CPR are required, with driving essential for home visits.

Qualifications

• Current and unrestricted RN licensure.
• Current CPR certification.
• Med surg/ICU/ER or acute care experience.
• Valid driver's license and reliable transportation.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Develop and implement individualized care plans with physicians.
• Monitor patients' conditions and report changes.
• Educate patients and families on disease management and treatment options.
• Maintain accurate records and coordinate with other professionals.
• Report progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure documentation is completed.
