Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse to provide one-on-one in-home nursing care, manage care plans, and work with a multidisciplinary team to support patient recovery at home.

You will administer medications, perform wound care, assess patients, and educate families on disease management. Strong collaboration skills and a valid driver's license are required for travel to patient homes.

Qualifications

Experience in med/surg, ICU, ER, or acute care settings.
Active, unrestricted RN license required.
CPR certification required.
Experience collaborating with healthcare professionals as part of a team.
Valid driver's license and reliable transportation.

Responsibilities

Provide skilled nursing care in patients' homes, including medications, wound care, and assessments.
Develop and implement individualized care plans with physicians and other professionals.
Monitor patient conditions and report changes.
Educate patients and families on disease management and medications.
Maintain accurate records and coordinate with healthcare team.
Report patient progress to physician and Clinical Manager.
Oversee Home Health Aides and ensure documentation completion.
