Job summary

Hueman is actively hiring Nurse Practitioners and Physician Assistants to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Elkton, VA.

Travel to patients’ homes, perform comprehensive non-invasive assessments, review medical history, document findings promptly, and provide basic wellness education. Must hold an active NP/PA license with national board certification and a Master’s degree.

Qualifications

• Master’s degree and licensure as NP or PA.
• Active state NP/PA license.
• Active national board certification.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure documentation.
• Conduct medical reconciliation for safety.
• Provide basic patient education on wellness.
