Qualifications

• Active, unrestricted license to practice medicine.
• Multi-state licenses preferred.
• Ability to cover Bridgeport and surrounding area.

Responsabilités

• The physician conducts in-home health evaluations for members served by Signify Health.
• Maintains patient safety and privacy during home visits.
• Documents assessments and coordinates with health plans and PCPs as required.
• Travels locally to client homes and may perform virtual visits as needed.
• Provides diagnostic & preventive services as indicated by the health plan.
