Job summary

Wabash General Hospital in Mount Carmel, IL is seeking a Community Paramedic / Prehospital Registered Nurse (PHRN) to provide advanced out-of-hospital care, conduct in-home assessments, and coordinate care across EMS, primary care, and hospital teams.

This role operates under established protocols to reduce avoidable hospital visits, improve access to services, and support population health initiatives, with opportunities for flexible scheduling and involvement in community health programs.

Qualifications

• Graduate of an accredited Paramedic Program or an accredited Registered Nursing Program.
• Community Paramedic Certification preferred or obtained within designated timeframe.
• Associate Degree in Nursing required; BSN preferred.
• Illinois Licensed Paramedic and/or Illinois Registered Nurse License.
• Current BLS, ACLS, and PALS Certification; Valid Driver's License.

Responsibilities

• Provide advanced in-home patient assessments, medication reconciliation, and patient education to promote self-management.
• Identify social determinants of health and connect patients with community resources and support services.
• Coordinate with EMS, primary care, hospitals, and other healthcare professionals to develop individualized care plans.
• Document assessments, interventions, and outcomes in the EHR; participate in performance improvement activities.
