Job summary
Wabash General Hospital in Mt Carmel, IL, is seeking a Community Paramedic / Prehospital Registered Nurse (PHRN) to deliver advanced in-home and community-based care. You will conduct comprehensive assessments, manage chronic conditions, reconcile medications, educate patients, and collaborate with EMS, primary care, hospitals, and social services to improve outcomes and reduce unnecessary hospital visits.
The ideal candidate will have graduate nursing or paramedic training, valid Illinois
Qualifications
- Graduate of an accredited Paramedic Program or an accredited Registered Nursing Program.
- Illinois Licensed Paramedic and/or Illinois Registered Nurse License.
- Current BLS, ACLS, and PALS Certification.
- May require evening, weekend, holiday, and on-call coverage.
Responsibilities
- Provide advanced out-of-hospital patient care and health assessments.
- Coordinate care across EMS, primary care, and hospitals.
- Perform medication reconciliation and patient education for self-management.
- Identify social determinants of health and connect patients with community resources.
- Document assessments and outcomes in the EHR.
