Job summary

Acadian Health is seeking an experienced Community Paramedic to provide hospital-level care in patients' homes through our Hospital-at-Home program. The role combines advanced clinical skills, care coordination, patient education, and technology to support recovery outside traditional hospital settings.

The ideal candidate is a licensed paramedic with BLS/ACLS, strong communication, and the ability to work independently with a remote care team.

Qualifications

• High School Diploma or GED required.
• State Paramedic license/certification required.
• BLS and ACLS certifications required.
• Valid driver’s license with clean record.
• Ability to obtain CP-C certification or equivalent within two years of hire.

Responsibilities

• Deliver hospital-level care in patients' homes for specified conditions.
• Coordinate care with physicians, nurses, and care teams.
• Educate patients and caregivers on medications, plans, and recovery.
