Job summary

CenterWell Home Health, a Humana Inc. company, is seeking a registered nurse to provide one-on-one home health care. You will administer admissions, develop and manage care plans, and coordinate with physicians and a broad care team to support patient recovery in the home setting.

The role requires an active nursing license, strong organizational skills, and reliable transportation. Prior home health or acute care experience is advantageous, with CPR certification preferred.

Qualifications

• Active RN licensure in good standing.
• Minimum of one year nursing experience preferred.
• Experience in med-surg, ICU, ER or acute care is a plus.
• Home Health experience is a plus.

Responsibilities

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Confer with physician to develop initial plan of treatment based on orders and assessment.
• Provide hands-on care and evaluation of the care plan under supervision.
• Coordinate care with clinicians and external providers as needed.
• Document patient care, progress, and changes; discharge patients after physician/manager approval.
