Job summary

Humana Inc. through CenterWell Home Health seeks a dedicated Home Health Registered Nurse to deliver one-on-one, in-home nursing care. You will collaborate with physicians to manage care plans, assess conditions, and guide patients toward independence.

You will educate families, document care meticulously, and supervise Home Health Aides while contributing to a patient-focused care team. A valid RN license and CPR are required.

Qualifications

• Current and unrestricted RN licensure required.
• Current CPR certification.
• Experience collaborating with a healthcare team.

Responsibilities

• Provide skilled nursing care in patients' homes, including medications, wound care, and assessments.
• Develop and implement individualized care plans with physicians.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management and treatment options.
• Maintain accurate care records and coordinate with healthcare professionals.
• Report care progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
