Family Home Health Network in the Chicagoland area is seeking a dedicated Home Health nurse to provide attentive patient care across home settings. You will perform initial assessments of patients, families, and environments, monitor ongoing nursing needs, and adjust the care plan as required to improve outcomes.

The ideal candidate holds a BSN with an active Illinois RN license, current CPR, and a valid IL driver’s license with auto insurance.

Qualifications

• Must have a BSN and an active Illinois RN license.
• Home Health experience is preferred but not required.
• Current CPR certification is required.
• Valid IL driver’s license and auto insurance are required.
• HCHB experience is a plus.

Responsabilités

• Perform a detailed initial assessment of patient, family, and home environment to determine care needs.
• Monitor and re-evaluate patient nursing needs on an ongoing basis.
• Initiate and revise the plan of care as patient status changes.
• Deliver high-quality, compassionate home health nursing services.
