Family Home Health Network in Illinois is seeking a dedicated Hospice Nurse with a strong commitment to quality patient care. You will perform a detailed initial assessment of the patient, family, and home environment to determine care needs and initiate the plan of care as patient status changes.

This role covers all home patients in the territory and requires excellent clinical nursing and customer service skills, IL RN licensure, CPR, and a valid driver's license; hospice experience is

Qualifications

• Must have IL RN licensure.
• Hospice experience preferred.
• Valid CPR and driver’s license.
• HCHB EMR experience is a plus.

Responsibilities

• Perform initial assessment of patient, family, and home environment to determine care needs.
• Monitor and re-evaluate patient nursing needs and adjust care plan as status changes.
• Initiate and revise the plan of care to ensure quality patient outcomes.
