Job summary

Family Home Health Network in the Chicagoland area is seeking a dedicated Home Health Nurse. The role involves performing a thorough initial assessment of the patient, family, and home environment to determine care needs, and then monitoring and adjusting the plan of care as patient status evolves.

Strong clinical nursing and customer service skills are essential, along with an Illinois RN license and a valid driver’s license. A comprehensive benefits package accompanies a full-time position.

Qualifications

• Requires Illinois RN licensure and CPR certification.

Responsibilities

• Perform a detailed initial assessment of the patient, family, and home environment to determine home health care needs.
• Monitor and regularly re-evaluate the patient's nursing needs and update the care plan as status changes.
• Initiate and revise the plan of care in collaboration with family and care team.

Family Home Health Network, is a provider of intermittent Home Health care, palliative care, hospice care and skilled/custodial private care services throughout the Chicagoland area.

We are seeking a dedicated Home Health Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the home environment to determine specific home health care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.

• Salary determined on experience and licensure

Qualifications:

• Must possess current Illinois RN licensure
• Home Health experience preferred
• Current CPR
• Valid IL driver’s license and auto insurance policy
• HCHB EMR experience a plus
• Must possess excellent clinical nursing and customer service skills
