Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations. The role focuses on connecting health plan members to the right care in their own homes, with a visit length of about 45 minutes and no requirement to prescribe or alter treatment.

Responsibilities include in-home assessments, review of meds and history, and occasional diagnostic tests as ordered by health plans.

Qualifications

• Active, unrestricted licenses in coverage area(s).
• Board certification is required.
• Multistate licenses are preferred.
