Job summary

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (NP or PA) to provide in-home health evaluations in Penobscot, ME. Visits last about 45 minutes, including medication/medical history review, physical evaluation, and potential additional tests as ordered by the health plan.

You will work with a national network of clinicians and may travel locally and within licensed states. The role requires active NP/PA licensure, board certification, and flexibility for virtual visits.

Qualifications

• Active, unrestricted license in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Provide in-home health evaluations to patients in their homes.
• Engage with patients to review medications and medical histories.
• Perform physical evaluations and order tests as indicated by health plan.
• Utilize iPad-based clinical workflow for evaluations.
• Travel locally within licensed states as needed.
• Conduct virtual visits when required.
