Job summary

Vetted Solutions is seeking a Nurse Practitioner or Physician Assistant (PRN) to perform in-home health assessments for health plan members in Maine. You will complete histories, physical exams, and risk assessments, and identify care gaps and social determinants affecting health outcomes.

You will document clinical findings in EMR, educate patients and caregivers on health status and care plans, and collaborate with physicians and care teams to support coordinated care and better patient
