Job summary

Matrix Medical Network is seeking a Nurse Practitioner or Physician Assistant (PRN) to perform comprehensive in-home health assessments for health plan members. You will take detailed histories, conduct physical exams, and perform risk assessments to identify gaps in care and social determinants that affect health outcomes.

You will document findings in EMR systems, educate patients and caregivers, and collaborate with physicians and health plans to improve care delivery.

Qualifications

• You must hold an NP or PA license in the US and be in good standing.
• Experience conducting in-home assessments and risk stratification is preferred.
• Ability to educate patients and caregivers and coordinate with physicians.
