DayToDay Health is recruiting a Nurse Practitioner to perform in-home health assessments and develop evidence-based care plans that improve patient outcomes. You will collaborate with an interdisciplinary team to close care gaps and coordinate care, while educating patients and caregivers on disease management and self-care strategies.

Responsibilities include documenting assessments and care plans in an EHR, supporting quality improvement, and contributing to cost-containment initiatives
