Job summary

CenterWell Certified Healthcare Corp. seeks a compassionate Home Health Aide to help patients stay in their homes by monitoring health, providing personal care, and teaching families how to assist with daily activities.

You will perform bathing, dressing, grooming, simple exercises, and maintain a clean, safe environment while adhering to Florida background checks and Humana's driver safety program. A valid Florida driver’s license and 1+ year of related experience are required.

Qualifications

• HHA 75 Hour Certificate or CNA State License is required.
• CPR certification required.
• OSHA, HIV/AIDS, HIPAA, Alzheimer's, Domestic Violence certificates required.
• Minimum 1+ year experience supporting individuals with disabilities, mental illnesses, or challenging behaviors.
• TB screening as part of Humana's TB program.
• Valid in-state driver’s license with registration and insurance; driving record with ≤2 points in last 3 years.
• Must have personal vehicle liability insurance (100,000/300,000/100,000).
• Florida Level 2 background screening per Florida Clearinghouse.

Responsibilities

• Keep patient environment clean and safe.
• Monitor and record patient condition; provide personal care (bathing, dressing, grooming).
• Teach families use of cane/walker and other equipment; assist with nutrition and hygiene.
• Help family lift/ reposition the patient and advise on safety.
• Record patient information in the journal and notify nursing supervisor of changes.
• Maintain asepsis and follow home care policies and state/federal requirements.
