Job summary

Signify Health, part of CVS Health, is seeking board-certified Physician Assistant/Physician Associate contractors to conduct in-home health evaluations for members in Medicare Advantage, Medicaid, and certain commercial plans.

These visits involve medication and medical history review, plus a physical examination performed in the member's home. We provide patient-specific forms and the role offers flexible hours without prescribing medications or ordering labs.

Qualifications

• Board-certified Physician Assistant with active license (PA-C).
• Experience performing in-home health evaluations or similar home-based assessments.
• Comfort traveling to member homes and performing home visits.

Responsibilities

• Conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.
• Review medication history and prior medical history; perform physical evaluation in patient's home.
• Do not prescribe medications or order labs; provide observations and plan recommendations to care teams.
