Signify Health, Inc. seeks board-certified physician assistants/physician associates (PA) to perform in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members.

The role involves patient visits in the home, reviewing medications and medical history, and conducting physical evaluations. PAs in this role do not prescribe medications or order labs, nor modify treatment plans.

Responsibilities

• Perform in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members.
• Review patient medications and medical history during visits.
• Conduct comprehensive physical examinations in the patient's home.
• No prescribing of medications, ordering labs, or modifying existing treatment plans.
