Job summary

Signify Health, a CVS Health division, seeks board-certified physician assistants to conduct in-home health evaluations for members of Medicare Advantage, Medicaid, and selected commercial plans. You will review medications and medical history and perform a physical assessment in the member's home.

Contractors receive a pre-filled form for each evaluation and will not prescribe medications, order lab tests, or modify existing treatment plans.

Qualifications

• Board-certified PA/PA-C with an active license.
• Experience conducting in-home health evaluations is preferred.
• Familiarity with Medicare Advantage and Medicaid guidelines.

Responsibilities

• Perform in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plans.
• Review medications and medical history; conduct physical assessments in patients' homes.
• Complete patient-specific forms; do not prescribe medications or order labs.
