Job summary

Blue Ridge Care is seeking a Registered Nurse (RN) for the PACE program in Winchester, Virginia. The RN will provide in-home clinical care to older adults, conducting assessments and implementing care plans while supporting their independence.

Qualifications

• Minimum of one year of experience working with the frail or elderly population.
• Experience with home health, geriatrics, hospice, or community-based care preferred.
• Medical clearance for communicable diseases and up-to-date immunizations prior to participant contact.

Responsibilities

• Conduct comprehensive in-home nursing assessments and ongoing evaluations of participant status.
• Perform skilled nursing visits including medication management and chronic disease monitoring.
• Develop, implement, and monitor individualized care plans in collaboration with the interdisciplinary team.
