Job summary

Appalachian Agency for Senior Citizens is seeking a Registered Nurse to support the PACE program in Marion, VA. The role involves nursing care, assessments, and participation in care planning to help frail elders live safely in the community.

You will work within an IDT, collaborate with teammates, and ensure compliance with state and federal requirements for direct patient care. A nursing license, an ADN, and valid driver’s license are required.

Qualifications

• Assists frail/elderly populations with nursing care and planning.
• Participates in care planning and IDT discussions.
• Demonstrates professional communication with patients and team.

Responsibilities

• Initial and ongoing assessment of health and nursing needs of PACE participants.
• Participates in planning, implementing and evaluating care plans in the PACE model.
• Collaborates with interdisciplinary teams to meet patient/family needs.
• Involves the healthcare team to ensure safe community living for elders.
