Job summary
FOX Rehabilitation is growing in Ottumwa, IA, and is seeking licensed Physical Therapist Assistants to deliver one-on-one, in-home care through FOX’s Geriatric House Calls™ model. You’ll work closely with a supervising PT to guide patients through personalized exercises, document visits in FOX’s EMR, and coordinate with the interdisciplinary team to support independence and mobility in older adults.

Qualifications
- Valid Physical Therapist Assistant license in state of practice.
- Graduate of an accredited PTA program.
- Current CPR certification.
- Basic computer skills including Microsoft Office.
Responsibilities
- Deliver individualized treatment plans established by supervising PT, adjusting activities based on patient response and progress.
- Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities.
- Document visits accurately in FOX's EMR system per Clinical Documentation standards.
- Coordinate with patients, POAs, and community referral sources to ensure coverage.
