FOX Rehabilitation is seeking clinicians to provide in-home and senior living care, delivering one-on-one physical therapy to older adults. You’ll evaluate, plan, and execute evidence-based treatments, documenting in our EMR and collaborating with PTs, OTs, SLPs, and Fitness Specialists.

Telehealth options expand flexibility, with a strong supportive culture and growth opportunities in New York. Base pay ranges in New York are competitive for full-time roles, with opportunities to earn more

Formación

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilidades

• Provide one-on-one physical therapy to older adults in homes or communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively in FOX’s EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate.
