Job summary

Fox Rehabilitation in Grass Valley, CA is seeking a licensed Physical Therapist to join our Geriatric House Calls model, delivering one-on-one care in private homes or senior living settings. Build strong patient relationships with autonomy over your schedule and treatment plans.

You will evaluate patients, develop individualized care plans, and provide evidence-based therapy while collaborating with an interdisciplinary team.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver sessions that support wellness, mobility, and independence.
• Document effectively within FOX’s EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
