Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in their homes and communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions supporting patient wellness and mobility.
• Document effectively within FOX’s EMR system.
• Collaborate with an interdisciplinary team.
• Provide telehealth/virtual care when appropriate.
