Job summary

FOX Rehabilitation is growing in Lafayette, LA and seeks licensed Physical Therapists to deliver one-on-one care to older adults in homes and Senior Living Communities. You will create and manage your own treatment schedule, using the Geriatric House Calls model to improve mobility and quality of life.

The role offers autonomy, flexibility, and meaningful patient outcomes, with opportunities to incorporate telehealth and virtual care to extend access and continuity of care.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes (private residences, senior living communities).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility, independence, and functional outcomes.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.

Education

Physical Therapy degree

PT license in state of practice or eligible
