Job summary

Scylla Solutions LLC is seeking a licensed Physical Therapist to provide outpatient rehabilitation services in an in-home setting for older adults in Seattle, WA. The role involves evaluating and treating in the patient’s home or in independent living facilities to promote independence and mobility.

The PT will educate patients and caregivers, coordinate with the rehab team, and adapt plans of care based on patient needs. A flexible schedule in various home environments is offered.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy or equivalent required.
• Master's degree in Physical Therapy related field with PT Bachelor also acceptable.
• Bachelor's degree in Physical Therapy plus 5 years progressive PT experience.
• PT degree earned outside the US must be evaluated for US equivalency.
• Must have reliable transportation to patients' residences.

Responsibilities

• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as per licensure.
• Adhere to department and patient treatment schedule; complete clinical and billing documentation.
• Attend and contribute to patient care and staff meetings.
• Refer to other disciplines to address patient needs.
• Collaborate with rehabilitation team to select appropriate therapy regimen.
• Adapt plan of care to individual patient needs.
• Promote safe environment and remove hazards; order supplies as needed.
• Maintain and repair equipment; ensure cleanliness and readiness.
• Deliver high-quality service with patient- and family-centered care.
• Screen, examine and evaluate patients; establish diagnosis and plan of care; document findings.
• Develop goals and methods with patients and caregivers; progress treatment plan.
• Incorporate health and wellness into therapy and perform home/community mobility assessments.
