Job summary

Girling Health Care of NY is seeking an experienced Occupational Therapist to conduct home visits in Manhattan, NY. The OT provides therapy per a physician's plan of care, assessing patient status and contributing to the total plan of care.

Responsibilities include initial and periodic assessments, developing a plan of care with measurable goals, documenting progress, and educating patients and caregivers. Licensure in NY and OT degree required; two years of experience preferred.

Qualifications

• This role requires licensure as an Occupational Therapist in New York State and an approved OT education.
• Experience in home care or institutional settings preferred.
• Ability to perform assessments, develop plans of care, and document progress.

Responsibilities

• Assess and evaluate therapeutic/rehabilitative/functional status.
• Develop and implement a plan of care with measurable goals.
• Provide OT treatment and collaborate with physician on plan of treatment.
• Document patient responses and adjust goals as needed.
• Educate patient, family, and home care staff on safe mobility and function.
• Complete case conferences, discharge summaries, and required forms.
• Participate in team meetings and QA programs.
