RVNAhealth seeks a Geriatric Care Manager to support older adults and families across Fairfield County. The role emphasizes aging safely at home, advocacy, and guiding complex care decisions in aging, chronic illness, and dementia.

Ideal candidates bring MSW/LCSW credentials in CT with CGCM certification, field-based experience, and strong communication to engage with clients, families, and clinicians.

Qualifications

• MSW required and LCSW in CT preferred.
• CGCM certification required or within six months of hire.
• RN or LPN with CGCM certification considered.
• NACCM certification acceptance or ability to obtain within six months.

Responsibilities

• Conduct psychosocial and care assessments in clients' homes.
• Develop individualized care plans addressing medical, emotional, and social needs.
• Coordinate healthcare services and community supports.
• Provide guidance and counseling to families navigating caregiving challenges.
• Advocate for client preferences and quality of life.
• Support dementia, chronic illness, and aging transitions.
• Collaborate with physicians, home health clinicians, and community partners.
• Participate in interdisciplinary care conferences and program development.
