21st Century Home Health Services is seeking an entry-level Field Physical Therapist to build core clinical skills in a home health setting, visiting patients in their homes. The role emphasizes follow-up visits, with strong leadership support, training, and mentorship.

The position reports to a Physical Therapist Case Manager and covers the El Dorado Hills area, with startup focus on building clinical skills and understanding company operations. California PT licensure and BLS/CPR are required.

Qualifications

• Current, unrestricted California Physical Therapist license.
• BLS/CPR certification required.
• Valid CA Driver’s License with clean driving record.
• Graduate of an accredited Physical Therapy program.
• One year of PT experience preferred; new grads considered.
• Home health experience highly desirable.
• Experience with high acuity patients is a plus.
• Able to collaborate in an interdisciplinary team (RN/OT/ST/MSW/HHA).
• Familiar with case management principles.
• Strong clinical judgment and communication skills.
• Proficient with electronic documentation and mobile tools.
• Self-directed, time management and professional demeanor.
• Commitment to annual compliance training and continuing education.

Responsabilités

• Provide hands-on interventions: gait/balance/ neuromuscular re-education, transfer training, therapeutic exercises, manual therapy, pain management.
• Assist patients with devices, prosthetics/orthotics, wheelchairs, and home safety equipment.
• Conduct detailed home evaluations and assessments.
• Implement plans of care under PT Case Manager supervision.
• Document care accurately within 24–48 hours per plan of care.
• Educate patients and families on mobility, exercises, safety, and adaptive equipment.
• Collaborate with PT Case Manager for treatments and care plan updates.
• Coordinate with RN/OT/ST/MSW/HHA for interdisciplinary care needs.
• Utilize equipment cost-effectively and maintain supplies.
• Maintain ongoing professional development and trainings.
• Perform other duties as assigned by Case Manager.
• Stay updated on patient communication and prioritize care accordingly.
• Provide updates to PT Case Manager and participate in case conferences.
• Ensure patient privacy and dignity in care and documentation.
• Seek opportunities to advance knowledge and adhere to best practices.
• Meet productivity expectations as directed by Case Manager.
• Assist in initial evaluations, reassessments, and discharge planning.
• Participate in on-call or weekend coverage as required.
