Advantmed is seeking a licensed Nurse Practitioner to perform structured in-home wellness and risk-adjustment assessments across diverse patient populations. This PRN role offers flexible scheduling, strong operational support, and autonomy in a professional setting.

Expect competitive per-visit compensation plus a $5,000 sign-on bonus, with paid mileage and a dedicated coordinator. Daily travel within a 55-mile radius, ongoing availability, and a commitment of about 30 hours per month are

Qualifications

• Active NP license in the applicable state.
• 3+ years of patient care experience preferred.
• Familiar with EMR systems and able to work independently.
• In-home or risk adjustment experience preferred.
• Willing to obtain additional licensure if needed (supported by Advantmed).

Responsibilities

• Conduct structured, in-home wellness and risk-adjustment assessments for diverse patient populations.
• Maintain consistent monthly availability and reliable on-site visits.
• Travel within a 55-mile radius of the assigned county.
• Operate with autonomy and professional accountability in the field.
