Qualifications

• Minimum of two years of post secondary education and at least two years of experience working with children and families.

Responsibilities

• Provide in-home support and teach practical skills to strengthen families.
• Connect families with community resources such as schools, healthcare, and social services
• Offer crisis intervention and guidance when needed
• Build strong relationships with families and collaborate with teachers, doctors, and social workers
• Develop service plans, monitor progress, and maintain documentation
