Job summary

Florida Department of Health is seeking a HUMAN SERVICES COUNSELOR I to support families with infants in a home-visiting program. You will coordinate care, document progress, and connect families to community resources.

Responsibilities include conducting initial and ongoing assessments, developing family goals, and supporting prenatal and infant health care. Flexibility to work evenings/weekends is requested.

Qualifications

• Motivation to learn and apply knowledge of child and family development.
• Basic counseling skills and rapport building with families.
• Ability to document client interactions electronically within the program.
• Flexibility to work evenings and weekends as required.
• Strong communication skills, both written and verbal.

Responsibilities

• Provide home visiting and case management for pregnant women and families with infants up to 5 years old.
• Coordinate care with an interdisciplinary team and community supports.
• Document progress in HMS and manage service ledgers timely.
• Meet grant and program requirements; track and report outcomes.
• Attend supervision, staff meetings, and mandatory trainings.
