NorthCare is seeking a Family Preservation Specialist to deliver in-home social services to families referred through DHS, focusing on reunification when possible. The role emphasizes culturally sensitive practice, weekly family visits, and collaboration with schools and agencies.

Training in SafeCare is provided to support parenting and home management skills. Responsibilities include intake, documentation, and developing intervention plans, with a requirement for a 40-hour work week and travel

Qualifications

• Bachelor’s degree in social work or related field.
• Master’s degree in social work or related field preferred.
• National SafeCare Home Visitor Certification required.
• Knowledge of family systems, cultural diversity, and community resources.
• Experience in assessment and intervention is desired.

Responsabilités

• Provides home-based social services in a respectful, culturally sensitive manner.
• Ensures weekly client contact and adequate service intensity based on safety needs.
• Advocates for families with schools and community agencies.
• Performs intake duties and completes documentation within required timelines.
• Conducts family assessments, develops intervention plans, and provides reunification services when applicable.
• Coordinates services with other providers to maintain community relationships.
• Implements SafeCare models to support parenting and home management skills.
