NorthCare seeks a Family Preservation Specialist to deliver in-home and community-based services to families referred through Oklahoma Human Services. You will coordinate complex cases, conduct multi-level family assessments, and develop intervention plans using evidence-based practices such as motivational interviewing and the SafeCare curriculum.

The role requires a Bachelor’s degree in a related human services field and at least one year of related experience.

Formación

Bachelor’s degree in a related human services field and one year of experience in a related practice setting.
Proficiency in MS Word and intermediate computer skills.
Ability to communicate effectively with English-speaking patients and families.

Responsabilidades

Coordinate complex cases using evidence-based interventions including motivational interviewing and SafeCare.
Provide mentorship to colleagues and support intake and case coverage.
Maintain confidentiality and professionalism while representing the program in the community.
Conduct in-home and community visits, ensuring at least weekly contact with families.
Complete intake within 24 hours, progress notes within 72 hours, and planned/discharges per policy.
Collaborate with schools, public/private agencies, and other providers to support families.
