Job summary

NorthCare is seeking a Family Preservation Specialist to provide community-based services, focusing on keeping children in their homes or aiding reunification after out-of-home placements. You will work with families using evidence-based strategies like SafeCare and motivational interviewing.

Responsibilities include home visits, rapid intake and progress documentation, and coordinating with schools and agencies to ensure continuity of care.

Qualifications

• Bachelor’s degree in a related human services field and at least 1 year of related practice experience.
• Experience coordinating complex cases using evidence-based interventions.
• Must be able to communicate effectively with English-speaking patients and families.

Responsibilities

• Conduct services in a natural environment convenient to the family.
• Maintain a minimum of one home visit per week, with flexibility for safety needs.
• Complete intake documentation within 24 hours and progress notes within 72 hours.
• Coordinate with schools, agencies, and providers to support family reunification and services.
