Job summary

Sevita's Family Centered Clinician in Home Therapist role serves Lowell and surrounding areas, Monday-Friday with day and after-school visits. You will provide person-centered assessments, develop goal-oriented treatment plans, and collaborate with referral agencies to support individuals and families.

Responsibilities include delivering evidence-based interventions, ensuring confidentiality, coordinating care with community partners, and offering aftercare planning.

Qualifications

• Master's degree in Social Work, mental health counseling, or related field required.
• Two years related field experience preferred.
• Exceptional communication skills with an ability to establish trust quickly.

Responsibilities

• Provide person-centered assessments in behavioral health, education, personality, and family interactions.
• Develop service plans based on assessment and collaboration with referral agencies and the individual served.
• Deliver activity- and participation-based interventions and document progress with families.
• Provide 24/7 coverage for all assigned individuals served and/or families.
• Make referrals to services and resources on behalf of individuals and/or families served.
• Maintain confidential, organized records and submit timely reports to supervisor.
• Participate in team meetings and support professional development of peers.
