Sevita is seeking a Family Centered Clinician - In Home Therapist to deliver person-centered assessments and treatment planning for individuals and families in Leominster and surrounding areas. You will develop goals, provide home-based interventions, and coordinate with referral agencies and community partners.

New graduates are welcome. The role offers a $6,000 new hire bonus and a salary range of $68,000–$75,000 with a monthly bonus opportunity.

Qualifications

• Master's degree in Social Work or related field required.
• Two years related field experience preferred.
• Exceptional communication skills with ability to establish trust and rapport quickly.
• A good listener with empathy while providing guidance.

Responsabilités

• Provide person-centered assessments in behavioral health, education, personality, community engagement, vocational needs, relationships, and family interactions.
• Develop service or treatment plans based on assessment, collaboration with referral agencies, and the individual served.
• Provide activity and participation-based interventions from training materials and family successes.
• Provide 24/7 coverage for all assigned individuals and families.
• Make referrals to services and resources on behalf of individuals and families served.
• Maintain confidential, organized documentation submitted to supervisor in a timely manner.
• Participate in team meetings with peers and collateral meetings when needed.
• Support peer development and address performance issues as needed.
• Provide an aftercare plan for each family/individual served.
• Build relationships with community partners and guide families in interacting with referrals.
