Job summary

Family Relief Resources in Portland, OR seeks a Direct Support Professional to assist a 15-year-old client with daily living, behavior support and community activities. Tasks include cooking, gardening, cleaning and safe transportation when needed.

Schedule is Wednesday–Friday 4:00 PM with flexible Saturday hours. Requires CPR/First Aid, background check, and smartphone-based timekeeping and reporting for shifts.

Qualifications

• 18 years or older
• Pass a Criminal History Check
• Work a minimum of 15 hours per month
• CPR and First Aid Certificate (within 3 months of hire)
• State required online training courses (pre-hire and within 3 months of hire)
• Continuing Education (24 hours per year)
• Basic knowledge of technology and smartphone timekeeping/reporting
• Experience with children and/or adults with varying support needs
• Confidentiality and safeguarding client information
• Able to communicate with health care providers, case managers and families
• Respond to emergency situations during service delivery

Responsibilities

• Care for a client on an individual basis in their home and community
• Teach independent living skills such as self-care, behavior management, budgeting, cooking, shopping and cleaning
• Not a facility-based role; all activities occur in the community or client home
• Track visits with GPS location verification via smartphone at clock in/out
• Have a valid driver’s license and car with insurance for transportation when needed
• Document progress notes after each shift and quarterly progress reports
• Maintain a set schedule and communicate effectively with client families and agency
• Exhibit patience, kindness and a positive attitude in all situations
• Be flexible to adapt to different personalities and disabilities
