Job summary

SuperCare Health in Modesto, CA seeks a Hospital Care Coordinator to organize DME and respiratory services for hospital and home settings. This non-sales role focuses on coordinating orders, documenting processes, and ensuring regulatory compliance across patient discharges and care transitions.

Under supervision of the Branch Manager, you will verify physician orders, coordinate equipment delivery, maintain records, and collaborate with internal teams to deliver timely, compliant care.

Qualifications

• Experience in medical supply, HME, or healthcare industry.
• Strong customer service and coordination skills.
• Knowledge of medical terminology and documentation.

Responsibilities

• Serve as the primary point of contact between SuperCare Health and hospital personnel for coordination of ordered equipment.
• Ensure all required documentation and physician orders are received, complete, and accurate and patient eligibility is verified prior to engaging with patient or family.
• Coordinate delivery of equipment to hospital rooms or patient homes in collaboration with internal teams and minimize duplication with other providers.
• Maintain accurate records of referrals, orders, and communications.
• Provide status updates to hospital staff regarding equipment delivery timelines.
• Maintain inventory levels in assigned hospital closets.
• Collaborate with Customer Care and Dispatch teams to ensure efficient service delivery.
• Attend all department meetings and training sessions.
• Maintain separation from hospital staff roles; act on behalf of SuperCare Health.
• Perform other duties as assigned by leadership.
