Job summary

Herewith is seeking an In-Home Dialysis Care Partner to support patients in Thousand Oaks, CA. You will assist with patient setup for dialysis, provide companionship, monitor safety, and coordinate with clinical staff.

A year of dialysis machine experience is required, and you will be vetted through the Herewith platform. The role offers consistent weekly hours at $25 per hour, with training and placement assistance and clinical team support to enable independent work in the client's home

Qualifications

• Prior experience with dialysis patients (home or clinical)
• Familiarity with home dialysis machines preferred
• Reliable, empathetic, and detail-oriented
• Strong interpersonal communication
• Must be dependable
• Must pass background check and vetting via Herewith platform

Responsibilities

• Set up and support patient setup for dialysis treatments (NxStage, Tablo or equivalent)
• Provide companionship, emotional reassurance, and ensure schedule adherence
• Monitor safety and assist with light ADLs as needed
• Maintain communication with clinic nurses and care coordinators
• Document concerns or changes observed during support hours
