Job summary

Herewith is seeking a compassionate Care Partner in Chesterland, OH to support a patient transitioning to home dialysis. This role provides one-on-one care with a single client, a stable 4-5 day weekly schedule, and training from our clinical team.

You’ll assist with NxStage setup, keep the home clean and safe, offer companionship, and monitor treatment times. Start ASAP; prior dialysis experience and the ability to complete training are required to join a dedicated home-care network.

Qualifications

• 1+ year of dialysis experience (home or clinic).
• Familiarity with NxStage and home dialysis systems is preferred.
• Willing to complete the provided training.
• Must pass Herewith background check.

Responsibilities

• Assist with dialysis treatment setup at home using NxStage machine.
• Keep the treatment area clean, organized, and safe.
• Provide companionship and emotional support.
• Maintain treatment schedule with the patient.
• Provide light ADL support as needed.
• Document simple notes and report concerns to the nurse.
