Job summary

Herewith is seeking a compassionate Care Partner to support a patient transitioning to home dialysis in Chesterland, OH. The role emphasizes one-on-one care, building rapport, and maintaining treatment schedules under clinical guidance.

Training will be provided for patient-specific dialysis procedures and home setup, with ongoing clinical team support to ensure confidence and safety throughout the transition.

Qualifications

• 1+ year of dialysis experience (home or clinic).
• Familiarity with home dialysis systems; NxStage experience is a plus.
• Willing to complete training and pass background check.
• Strong communicator who works well with a clinical team.

Responsibilities

• Assist with dialysis treatment setup (NxStage machines).
• Keep the treatment environment clean, organized, and safe.
• Offer companionship and emotional reassurance.
• Help the patient remain on schedule with treatment times.
• Provide light ADL support as needed.
• Monitor safety and report concerns to the coordinating nurse.
• Maintain simple documentation and shift notes.
