Job summary

Herewith is seeking a compassionate Care Partner to support a patient transitioning to home dialysis in Cornelius, NC. The role offers stable weekly hours with a single client and includes comprehensive training from our clinical team.

You will assist with home dialysis setup, maintain a clean and safe treatment space, provide companionship, and help keep treatment times on schedule. Prior dialysis experience and strong communication are valued.

Qualifications

• 1+ year dialysis experience (home or clinic).
• Familiarity with home dialysis systems (NxStage, Tablo) preferred.
• Open to in-home dialysis support and willing to complete training.
• Steady, supportive, patient—especially during adjustment period.
• Strong communicator who works well with a clinical team.
• Must pass a Herewith background check.

Responsibilities

• Assist with dialysis treatment setup (NxStage or Tablo dialysis machine).
• Keep the treatment environment clean, organized, and safe.
• Offer companionship and emotional reassurance.
• Help the patient remain on schedule with treatment times.
• Provide light ADL support as needed.
• Monitor safety and report concerns to the coordinating nurse.
• Maintain simple documentation and shift notes.
