Job summary

Herewith is seeking a compassionate, reliable Spanish-speaking Care Partner to support in-home dialysis for a patient transitioning from clinic care. This is a one-on-one, consistent assignment with comprehensive training from our clinical team.

You will help set up in-home dialysis (NxStage or similar), maintain a safe treatment space, and ensure treatments stay on schedule while providing companionship and support to the patient and family.

Qualifications

• Minimum 1 year of dialysis experience (home or clinic).
• Familiarity with home dialysis systems preferred (NxStage a plus).
• Fluent in Spanish (required).
• Open to in-home dialysis care and completing provided training.
• Calm, patient, and dependable—especially during care transitions.
• Strong communication skills and ability to collaborate with a clinical team.

Responsibilities

• Assist with in-home dialysis treatment setup (NxStage or similar systems).
• Maintain a clean, organized, and safe treatment environment.
• Support the patient in staying on schedule with dialysis treatments.
• Provide companionship and emotional reassurance.
• Assist with light Activities of Daily Living (ADLs) as needed.
• Monitor patient safety and report concerns to the coordinating nurse.
• Maintain simple documentation and shift notes.
