Herewith is seeking a compassionate Care Partner to provide in-home dialysis support for a patient transitioning from clinic to home care. This is a one-on-one assignment designed to build trust and continuity of care with ongoing clinical team support.

You will receive comprehensive training on patient-specific dialysis procedures, home setup guidance, and coordinated nursing support to ensure confidence and safety in this role.

Qualifications

• Minimum 1 year of dialysis experience (home or clinic).
• Familiarity with home dialysis systems preferred (NxStage a plus).
• Fluent in Spanish (required).
• Open to in-home dialysis care and completing provided training.
• Calm, patient, and dependable—especially during care transitions.
• Strong communication and ability to collaborate with a clinical team.

Responsibilities

• Assist with in-home dialysis treatment setup (NxStage or similar systems).
• Maintain a clean, organized, and safe treatment environment.
• Support the patient in staying on schedule with dialysis treatments.
• Provide companionship and emotional reassurance.
• Assist with light Activities of Daily Living (ADLs) as needed.
• Monitor patient safety and report concerns to the coordinating nurse.
• Maintain simple documentation and shift notes.

Compensation: $125 per treatment session

About This Role

We are seeking a compassionate, reliable, and fluent Spanish-speaking Care Partner to support in-home dialysis for a patient who currently receives treatments in a clinic and will transition to home dialysis. This is a one-on-one, consistent assignment (not an agency rotation), allowing you to build trust, rapport, and become an integral part of the patient's ongoing care routine. Comprehensive training will be provided, including patient-specific dialysis procedures, home setup guidance, and continued support from our clinical team, ensuring you feel confident and well-supported in this role.

This position is ideal for someone who values patient dignity, understands the complexity of dialysis care, and wants to make a meaningful impact during an important care transition.

Primary Goal
